Billy Joel says decades of loud concerts drove his NPH diagnosis and shunt surgery

The constraint in Billy Joel's case was acoustic: repeated high-decibel performances, his doctors told him, were delivering something equivalent to a concussion every time he took the stage. That cumulative load on his brain nerves, Joel disclosed on his SiriusXM program "The Billy Joel Channel," is what his medical team believes drove the normal pressure hydrocephalus diagnosis he made public in May 2025.

NPH occurs when cerebrospinal fluid accumulates inside the skull and presses on the brain. The Cleveland Clinic describes the condition as most likely to affect people over 65. Joel is 77. The shunt surgery his team recommended drains the excess fluid to relieve that pressure continuously. He described the surgical team's explanation in plain terms: a hole drilled through the skull, a shunt placed inside the brain.

He resisted initially. After reading about the procedure's outcomes, he agreed. The surgery improved his memory, his balance, and his general ability to function.

The concert-as-concussion problem

Joel told the SiriusXM audience that his first symptom was a persistent rocking sensation, as if he were on a boat, paired with unsteady footing. A neurologist gave him the NPH diagnosis and warned that the condition could be progressive.

The connection to his career came from his doctors directly. Playing rock music at high volume was producing concussion-equivalent events at every performance, they told him, and each show was causing further damage to his brain nerves. More performances meant more cell loss.

When Joel disclosed the diagnosis in May 2025, his team's statement said the condition had been "exacerbated by recent concert performances, leading to problems with hearing, vision and balance." He was advised to stop performing and to begin specific physical therapy.

On Bill Maher's podcast "Club Random," Joel told Maher the balance impairment was the most disruptive effect, again describing it as being on a boat. He speculated that alcohol might be a cause. His doctors, he said, offered no single clear explanation for why the NPH developed.

He is still in recovery. His medical team also instructed him to avoid stress. He has been following both directives and told his SiriusXM listeners that he is not willing to risk further cognitive loss by returning to the stage prematurely.
