Billy Joel says decades of loud concerts drove his NPH diagnosis and shunt surgery

The constraint in Billy Joel's case was acoustic: repeated high-decibel performances, his doctors told him, were delivering something equivalent to a concussion every time he took the stage.

That cumulative load on his brain nerves, Joel disclosed on his SiriusXM program "The Billy Joel Channel," is what his medical team believes drove the normal pressure hydrocephalus diagnosis he made public in May 2025.

NPH occurs when cerebrospinal fluid accumulates inside the skull and presses on the brain. The Cleveland Clinic describes the condition as most likely to affect people over 65.

The shunt surgery his team recommended drains the excess fluid to relieve that pressure continuously.
